How It Works

Estate Planning Made Easy

The Estate Plan Store® equips Arizona residents with seven essential legal documents:

  • Last Will and Testament - contains instructions as to how your property and assets will be distributed upon death, and designates a guardian for your minor children. Read more about Last Will and Testament.
  • Living Will - contains instructions about end-of-life decisions that would be invoked if you have a terminal condition or an irreversible coma or a persistent vegetative state. Without this document, the hospital or government may keep you alive indefinitely, all the while draining your family’s financial resources.
  • Springing General Power of Attorney - authorizes another person to manage your financial and business affairs if you become incapacitated. The Estate Plan Store® uses “springing” general power of attorney documents that “spring” into effect only if you become incapacitated.
  • Durable Medical Power of Attorney - an authorization to act on your behalf in regards to medical decisions if you are unable to express your wishes. Read more about Medical Power of Attorney.
  • Durable Mental Health Care Power of Attorney – helps avoid the need for a civil commitment court proceeding when a person suffers from mental illness and is a danger to him or herself or others. Such a hearing can be expensive, time-consuming and embarrassing.
  • HIPAA (Health Insurance Portability and Accountability Act) Authorization – authorizes designated persons the permission to obtain your health information if you become incapacitated. Without this document it is possible that your loved ones may not be able to obtain information about your medical condition if you are unable to communicate.
  • Funeral and Burial Instructions – contains instructions as to how you want your body to be treated upon passing.

These documents can easily run more than $1,000 if acquired traditionally through a law firm. Yet the Estate Plan Store® offers this entire package for as low as $495. All customers reap the same legal benefits found at trusted law firms, but they gain the added convenience and lower costs of an online store.

The process can be completed three ways:*
Online Submittal: $449

    Set up an account and review the estate plan checklist. It contains a list of documents you’ll need to gather before starting the process.

    Fill out the online questionnaire. You will be guided through a series of questions spanning your personal background, financial status, medical wishes and asset distribution plans. It should take about 30 to 45 minutes to complete. You can save your progress and return later if needed. If you have questions about the form, call (602) 466-1641.

  • 3
    All legal documents were created by experienced estate planning attorneys. Your specific estate plan will be prepared under the supervision of legal document preparers certified by the Supreme Court of Arizona. You will be notified by phone if clarification or changes are needed to any of the documentation.

    Within 10 business days, you’ll receive a call from the Estate Plan Store® with instructions on how to sign and pick up the completed estate plan.

In-Person Submittal: $395
  • 1
    Those who prefer to complete the process offline can call (602) 466-1641. Depending on your location, we will mail you a hard copy of the estate plan questionnaire and checklist, or you can pick up the documents in person. A 50% deposit is due at this time.

    You can complete the questionnaire at home or with assistance at the Estate Plan Store®. Full payment is due when you submit the completed form.

    The legal forms are created by experienced estate planning attorneys and prepared by legal document preparers certified by the Supreme Court of Arizona. You will be notified by phone if clarification or changes are needed to any of the documentation.

  • 4
    Within 10 business days, you’ll receive a call from the Estate Plan Store® with instructions on how to sign and pick up the completed estate plan.

Urgent Submittal: Prices Vary

A representative from the Estate Plan Store® can travel to a nursing home, hospital or another Arizona location when urgent circumstances require it. We will walk you through the documentation and notarize the forms on-site. To set up an appointment, call (602) 466-1641.
